This form is not an application for insurance and it is not personal advice.
My answers are complete and accurate to the best of my knowledge.
If I go ahead with a full application, the insurer will set out my obligations when answering their questions. For most personally held cover (a consumer insurance contract), this is a duty to take reasonable care not to make a misrepresentation to the insurer. For some other contracts, a duty of disclosure may apply instead. The insurer's own wording will appear on any later application, and I should read it carefully.
I authorise BMP Advisory and my adviser to use the information in this form, including health information, to seek indicative terms from one or more life insurers, and to discuss it with them for that purpose.
I understand that indicative terms are not an offer of cover. Any insurer's final decision depends on a full application.
BMP Advisory will handle my information in line with its privacy policy (available on request) and use it for this pre-assessment.
